Enrolling in the Plan

You can obtain enrollment materials from Coast Benefits, or from the IBEW Local 40 union hall.  The package contains extensive information on the plan, and enrollment forms.  Complete the enrollment forms and return it to us.

Wage Deferral Form. This form comes with your enrollment kit, but you may also download and fill out the Wage Deferral Enrollment Form.

This packet is sent directly from Putnam Investments.  They handle all your information, and you will be working directly with Putnam Investments.  Complete and send this packet to Putnam investments (not Coast Benefits at the address above).  If you did not receive a packet, you may request another packet here.
